UPDATE ON PRACTICE AND WELLNESS BUSINESS (abajo en Español)

Hello! Dr. Liz Cruz here, below is the update on my medical practice and wellness business.

Please take the time to read through everything below carefully as it should answer all your questions.

Due to many unforeseen circumstances, I have had to make some difficult decisions about the future of my medical practice. Although this is not how I planned to wrap things up, I truly believe God is giving me the push I needed to make a move I should have made long ago.

Most of you know, I have been offering a more holistic approach at my office for years. Although my staff primarily offered those services to my patients while I continued to scope, as that business grew, it had been calling me more and more. It is from the success of that business that I see people truly getting well and that is why I became a doctor to begin with. It is in that business that I want to put more of my efforts. Let me be clear, I DO NOT plan to retire, I just plan to slow down. I know many of you have mentioned your plan to follow me wherever I go, I welcome that and look forward to continuing your care.

As of May 1, 2025, after 18 years in business, I officially closed the doors to my office and merged it with Dignity Health Medical Group. Starting June 2025, I will be working out of their offices at 500 W. Thomas Road, Ste. 100, in Phoenix. Their phone number is (602) 406-1510 and fax number is (602) 406-7277. It will be a similar setup to what it was in my office. You will see a Nurse Practitioner in clinic and then schedule with me for procedures. Two days per week, I will be scoping out of the St. Joe's Outpatient Surgery Center located at 240 W. Thomas Road Phoenix, and one day per week I will be scoping at my current location of Surgical Elite of Avondale at 10815 W. McDowell Road Ste. 101, Avondale.

Unfortunately, Dignity does not accept all insurances, especially Banner insurance, so if you are one of my Banner patients you will need to find a new Gastroenterologist. If you are a new or established patient that has a new referral to our office, you must ask your referring doctor to resubmit the referral to Dignity Health Medical Group.  

If you are a patient that requires regular prescription refills you have three options as most of the medicines we prescribe are common medications. Option 1 - contact your primary care provider, Option 2 - call Dignity to get established with Dr. Cruz’s new location or Option 3 - get established with a new GI doctor as soon as possible. Depending on where you live, I have listed a few GI's here that I highly recommend:

• Southwest Valley - Dr. Raje Anaparthy at Southwest Gastroenterology - 623-322-0323
• Northwest Valley - Dr. Anthony Merrill at Arrowhead Gastrointestinal Associates - 623-376-6328 - they do take all Banner insurance except Banner AHCCCS
• Southeast Valley - Dr. Mary Atia at Arizona Digestive Health - 480-461-1088
• Northeast Valley – Dr. Daniel Meline at Arizona Digestive Health – 480-860-1990

Regarding Medical Records Requests - As of May 1, 2025, all medical records have been transferred to American Medical Solutions (the “Records Custodian”). The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be held for at least 7 years from the date of your last service, depending upon applicable state and federal record retention requirements. If you wish to have a copy of your records sent to you or to another provider or facility, you may submit your request to the Records Custodian anytime between May 2025 and 7 years from then.

To submit a request for records you must download, complete, print, and sign an Authorization of Release form. This form can be submitted in their office OR via the following methods:

Outstanding Balances

If you owe our practice money for the services we rendered to you, we have made a point to send multiple invoices along with one final letter informing you of your balance along with several ways to pay. If we did not hear from you after sending this final letter, we did send your outstanding balance to collections. The name of the collections company is Beyond Green Solutions. If you received a statement from them on our behalf and you have questions or would like to pay your outstanding balance, you can call them at 585-450-2775 or visit their website at www.beyondgreensolutions.com.

Many of you have asked about the plan for supplement purchases once the office closes.

THE PLAN FOR ORDERING SUPPLEMENTS

Starting May 1, 2025, here are the options for supplement orders:

1. We are now offering all our products on Amazon - you can order them anytime, day or night and if you have Prime - shipping is free. This is a great option for those of you purchasing only one or two bottles at a time. Here is a link to our store on Amazon – Dr. Liz Cruz Store.
2. If you prefer not to use Amazon and / or regularly purchase in bulk, for our bulk discounts, you can either order them on our website https://drlizcruz.com/- (we have a special option there to purchase the 5 or 12 bottle blends) or you can text or call Tina on the Dr. Liz Cruz Wellness Hotline (please save this number to your phone) 602-829-5656. You can either call or text in your order, whatever is easiest for you. These products will automatically get shipped but we are waiving all shipping fees on bulk orders going forward. Also, please don't be concerned about the probiotics being shipped. We have been shipping them for years and they do just fine in the mail for a few days (even in the hot AZ summer)
3. If you are determined to pick up your products and refuse to have them shipped (even for free), there will be a $5.00 fee to have them delivered to the Arrowhead Mail and More on the SE corner of 67th Avenue and the 101. Let us know when you are ordering, and we will let you know when they will be available for pick-up.
4. If you only use Cash to purchase your supplements, we do offer purchasing through Venmo, Paypal, Zelle, or Cash App. Let us know when you are purchasing, and we will coordinate whatever form of payment works best for you.

It has been an absolute blessing taking care of my patients since 2007. I will do what I can to assist everyone's transition to ongoing quality care. PS - Update on Maureen – Maureen was diagnosed with metastatic lung cancer on November 3, 2024. She died 5 months later, on April 3, 2025. We appreciate all the prayers and well wishes we have received on Maureen’s behalf as this is such sad news for our Dr. Liz Cruz family. I know many of you loved Maureen and even though some did not take to her Pittsburgh tough love approach, Maureen was an integral part of our life and our business. She consistently provided compassionate and loving care to our patients and was very knowledgeable in GI. She was always very concerned about the patient’s well-being; we couldn’t have done any of it without her and will always appreciate the expertise she brought day in and day out. Please continue to pray for her family that they find peace and comfort in this difficult time.

PS: Actualización sobre Maureen: A Maureen le diagnosticaron cáncer de pulmón metastásico el 3 de Noviembre de 2024. Falleció 5 meses después, el 3 de Abril de 2025. Agradecemos todas las oraciones y buenos deseos que hemos recibido en nombre de Maureen, ya que esta es una noticia muy triste para nuestra familia, la Dra. Liz Cruz. Sé que muchos de ustedes querían mucho a Maureen y, aunque algunos no aceptaron su estilo de duro de Pittsburgh, Maureen fue una parte integral de nuestra vida y nuestro negocio. Siempre brindó atención compasiva y amorosa a nuestros pacientes y tenía un amplio conocimiento en gastroenterología.
